Admissions Process

Hope House accepts applications online (primary method), in person, via fax or telephone, as well as physically mailed applications. It takes approximately two (2) to five (5) business days to process an application for services.

Step two

In addition to the application, Hope House will need the following items for you or your loved one to be considered for treatment:

TUBERCULOSiS TEST RESULTS

Also known as a TB or PPD test. The results of this test take three (3) days and can be completed with your physician or your local health department.

SYPHILIS test results

Also known as a RPR test. The results of this test take 24-48 hours and can be completed with your physician or your local health department.

Income verification

Also known as a WG-15 from the Georgia Department of Labor. This is a free form.

Photo identification

Also known as a picture ID. This can include a driver's license or state identification card. Photos of your ID Card are allowed-PLEASE MAKE SURE THE IMAGE IS CLEAR and email to docs@hopehouseaugusta.org.

social security card

This can be the physical card or document from the Social Security Administration stating that a replacement card/new card has been applied for. Photos of your Social Security Card are allowed-PLEASE MAKE SURE THE IMAGE IS CLEAR and email to docs@hopehouseaugusta.org.

Homeless verification (if applicable)

This can be a notarized letter from an emergency shelter, agency, church, and/or family member/friend that has provided services, including temporary shelter or hotel rental assistance. Include your last known address or zip code with this letter.

*If you complete a hard copy of the application, please also complete and sign the mandatory Augusta Housing Authority form. All forms and additional documents can be emailed to docs@hopehouseuagusta.org or faxed to 706.737.9830 with attention to 'Intake.'

Although many individuals that apply for services at Hope House are court ordered to participate, our agency follows guidelines set forth by the Georgia Department of Behavioral Health and Developmental Disabilities for services. All programs providing treatment for substance using women give priority for admission in the following order:

1. Pregnant, intravenous substance users.

2. Other pregnant substance users.

3. Other intravenous substance users.

4. All other substance users.

Court ordered individuals are not provided a priority level unless they indicate one of the above priority levels, as appropriate.

sTEP FOUR

Our services are provided through contracts, grants, and local donations. While our facility consistently maintains a waiting list for services, Hope House will work with you or your loved one to identify appropriate alternative treatment options, as well as provide interim services. With the above identified priority levels, an estimated wait time cannot be provided to you or your loved one.

In the meantime, you will receive a weekly call from our intake team or a certified peer on staff to provide an update on your admission's status and complete a recovery check in. If you do not hear from us, please ensure we have your correct contact information.

Need support outside of our normal business? Contact the CARES Warm Line at 1-844-326-5400. The CARES Warm Line is a peer-operated, non-emergency telephone support service provided in partnership with the Georgia Department of Behavioral Health and Developmental Disabilities (DBHDD). Telephone lines will be open 8:30 a.m. until 11:00 p.m. Services are free and all information will remain confidential.

The CARES Warm Line is not a crisis line. If someone is experiencing a mental health crisis or emergency, they will be referred directly to the Georgia Crisis and Access Line at 1-800-715-4225.
